BCVFA ePAT Application

Welcome to the Baltimore County Fire Service's Electronic Personnel Accountability Tag (ePAT) application system.  This system is for use by volunteer company officers in Baltimore County Only.  Company officers should complete one of the below forms and attach all supporting documentation as required.  Please ensure that all information is submitted as requested and accurately.  Incomplete or incorrect applications will be rejected and you will be required to submit another application in its entirety.  If you find a misprint on the PAT Tag or have a name change within six months of receipt of the TAGS please send an email to mliddick@baltimorecountymd.gov DO NOT submit a new application.

Initial Issuance or Downgrade - Silver Tags (Non-Entry FIRE)
Initial Issuance or Downgrade - Blue Tags (EMS Only)
Initial Issuance or Upgrade - White Tags (Entry)
Career to Volunteer Tag Request (White or Blue)
Broken or Damaged Tag Replacement (White, Silver or Blue)

Initial Issuance or Downgrade - Silver Tags (Non-Entry FIRE)

Company officers should utilize this form to request Silver (Non-Entry) tags for their personnel if they are applying for new tags (e.g. they have never had a set of PAT Tags) or if they are downgrading from White (Entry) PAT Tags.  This link should not be utilized for members requesting tags that have been broken/damaged, or are missing/stolen.  To apply, you will need the following items:

  1.  Demographic information for the member (name and LOSAP).
  2.  An electronic copy of the PAT Tag photo.  Due to the equipment which is utilized for PAT Tag production, it is important that a clear and well-lit photo be submitted in order to produce the best quality tag possible.  All images must include the individual centered in the middle of the photograph from the shoulders to a few inches above their head.  Images must be taken against a blue background and should be in good taste and professional.  Images which do not follow these guidelines will cause the application to be rejected.  The photo format should be one of the following formats: .jpg, .jpeg, .gif., .png, .tiff, or .eps.
  3.  A .pdf copy of proof of Hazardous Materials training.  If the member took the training within the past 12 months you only need to submit either A or B.  If the member's training is older than 12 months then you must submit a or b and c.
    1.  A copy of a certificate through the Baltimore County Fire Department Learning Management System for Hazardous Materials Core Competencies.  Please note that MFRI Hazmat Awareness is not acceptable for this requirement or
    2. A copy of MFRI Hazardous Materials Operations or Hazardous Materials Technician (MFRI course completion certificate or MFRI transcript)
    3. A copy of the most recent Hazardous Materials Operations Refresher from the Baltimore County Fire Department Learning Management System.  Both of these certifications can be attached.
  4.   A .pdf copy of proof of EMS or CPR/AED certification.  This should be one file and include one of the following options:
    1.  A current EMS licensure or certification issued by the State of Maryland (MIEMSS) - EMR, EMT, CRT, or Paramedic.  This must be the MIEMSS card; a MFRI transcript does not satisfy this requirement.
    2.  A current CPR/AED card; this must include the front and back of a current certification card or the entire e-card.  It must clearly state CPR and AED and must be issued by a recognized institution whose curriculum meets the current consensus standards for CPR and ECC as established by the International Liaison Committee on Resuscitation (ILCOR).  CPR must include hands-on.  It cannot be on-line only.
  5.  A .pdf copy of the most current version of the EMS Compliance Training certificate from the Baltimore County Fire Department Learning Management System.

Initial Issuance or Downgrade - Blue Tags (EMS Only)

Company officers should utilize this form to request Blue (EMS Only) tags for their personnel if they are applying for new tags (e.g. they have never had a set of PAT Tags) or if they are downgrading from White (Entry) PAT Tags.  This link should not be utilized for members requesting tags that have been broken/damaged, or are missing/stolen.  To apply, you will need the following items:

  1.  ALL items (1-6) from the Initial Issuance or Downgrade - Silver Tags (Non-Entry) listing above.

Initial Issuance or Upgrade - White Tags (Entry)

Company officers should utilize this form to request White (-Entry) tags for their personnel if they are applying for new tags (e.g. they have never had a set of PAT Tags) or if they are upgrading from Silver (Non-Entry) PAT Tags.  This link should not be utilized for members requesting tags that have been broken/damaged, or are missing/stolen.  To apply, you will need the following items:

  1.  ALL items (1-6) from the Initial Issuance or Downgrade - Silver Tags (Non-Entry) listing above with the exception that the Appendix A must indicate entry.
  2.  A .pdf copy of completion of Firefighter I training.  This should be one file and include one of the following options:
    1.  A MFRI certificate or transcript indicating Firefighter I or Firefighter II completion
    2.  A Pro Board/IFSAC certification or transcript indicating Firefighter I or Firefighter II certification.
    3.  An e-mail from the Chief Officer of the Baltimore County Fire-Rescue Academy indicating the member has passed Firefighter I.  Any other e-mails or proof of completion will not be accepted.

Career to Volunteer Tags (Current Equivalent Level)

Company officers should utilize this form to request PAT tags for their personnel that are currently Career Members of the Baltimore County Fire Department. This link should not be utilized for members requesting tags that have been broken/damaged, or are missing/stolen.  To apply, you will need only the basic demographic information for the member.  PLEASE NOTE:

  1. Members will be issued a tag for their current clearance level in the Career Department.  Therefore, if a member is recognized as a White (Entry) Tag on the career side, they will be issued a White (Entry) tag.  Similarly, career members recognized as a Blue (Non-Entry) tag on the career side will be issued a Silver (Non-Entry tag).
  2. Career members who are non-entry (Blue Tag) on the career side who wish to obtain entry (White Tag) status as a volunteer MUST complete the Initial Issuance or Upgrade - White Tags (Entry) FULL APPLICATION.

Broken or Damaged Tag Replacement (Silver, Blue & White)

Company officers should utilize this form to request replacement tags for their personnel.  The tags must be in the member's possession and be turned back in when new tags are received.  Tags will be re-printed for the level previously approved.  This link should not be utilized for members requesting replacement tags that are missing or have been stolen.  To apply, you will need all of the following items.  Failure to attach all items will result in the request being delayed or denied.

  1.  Demographic information for the member (name and LOSAP).
  2.  Number of Tags to be replaced
  3. .pdf copy of proof of EMS or CPR/AED certification.  This should      be one file and include one of the following options:
    1.  A current EMS licensure or certification issued by the State of Maryland (MIEMSS) - EMR, EMT, CRT, or Paramedic.  This must be the MIEMSS card; a MFRI transcript does not satisfy this requirement.
    2.  A current CPR/AED card; this must include the front and back of a current certification card or the entire e-card.  It must clearly state CPR and AED and must be issued by a recognized institution whose curriculum meets the current consensus standards for CPR and ECC as established by the International Liaison Committee on Resuscitation (ILCOR).
  4.  A .pdf copy of the most current version of the EMS Compliance Training certificate from the Baltimore County Fire Department Learning Management System.
  5.  A .pdf copy of the most current version of the Hazardous Materials Operation Refresher certificate from the Baltimore County Fire Department Learning Management System.

Missing or Stolen Tag Replacement (Silver, Blue or White)

Company officers should utilize this form to request replacement tags for their personnel if they are missing or stolen.  Tags will be re-printed for the level previously approved.  This link should not be utilized for members requesting replacement tags that are broken or damaged.  To apply, you will need the following items:

  1.  Demographic information for the member (name and LOSAP).
  2.  Number of Tags to be replaced.
  3.  A .pdf copy of a police report in the jurisdiction where the loss or theft occurred.
  4.  A brief explanation of the circumstance leading to the loss or theft (this may be completed in the form itself).
  5. .pdf copy of proof of EMS or CPR/AED certification.  This should be one file and include one of the following options:
    1.  A current EMS licensure or certification issued by the State of Maryland (MIEMSS) - EMR, EMT, CRT, or Paramedic.  This must be the MIEMSS card; a MFRI transcript does not satisfy this requirement.
    2.  A current CPR/AED card; this must include the front and back of a current certification card or the entire e-card.  It must clearly state CPR and AED and must be issued by a recognized institution whose curriculum meets the current consensus standards for CPR and ECC as established by the International Liaison Committee on Resuscitation (ILCOR).
  6.  A .pdf copy of the most current version of the EMS Compliance Training certificate from the Baltimore County Fire Department Learning Management System.
  7.  A .pdf copy of the most current version of the Hazardous Materials Operation Refresher certificate from the Baltimore County Fire Department Learning Management System.

Volunteer ID Card

Company officers should utilize this form to request a Volunteer ID Card for their personnel.  This link should not be utilized for members requesting any type of PAT Tags.  To apply, you will need the following items:

  1.  Demographic information for the member (name and LOSAP).
  2.  A current photo of the member.
